For new patient appointments, we require patients to arrive 30 minutes prior to their appointment to fill out paperwork and update information. Alternatively, new patients can download the New Patient Packet from the forms section on this page to fill out prior to their appointment. New patients MUST bring a photo ID, all insurance cards, and any medications they are taking with them to their first appointment.

We at LifeLink pride ourselves at minimizing wait times while treating our patients. If you are over 15 minutes late to your scheduled appointment, we will have to reschedule you to a later time or date. If you need to cancel an appointment, we ask that you give us 24 hour notice so that we can give the opening to another patient in need. There is a $25.00 charge for any missed appointments, with increased fees for missing imaging appointments scheduled at our office.

All insurance changes must be given to us at time of service. Copayments are collected when you check-in for your appointment. If you are unable to pay your copay, we will have to reschedule your appointment. We do accept checks for insurance copayments, with a $25.00 fee for any returned checks.

If you require a refill on any of your medications, contact your pharmacy first. Refills requested will take 24 to 48 hours to process. Some refills will require an office visit in order to renew. There has also been an increase in the number of medications that require prior authorizations from your insurance in order to fill, which will extend the time it takes to fill.
